چکیده

Atrial septal defect (ASD) is the most frequently diagnosed congenital heart disease in adulthood. Along with the recent evolvement of therapeutic catheterization, transcatheter closure has become the treatment of choice in most secundum type ASDs in children and adults. However, clinical decision and technical details may be influenced by special circumstances including complex morphology of the defect, co-morbid disease, significant arrhythmia, pulmonary arterial hypertension (PAH), tricuspid or mitral insufficiency, ventricular dysfunction, and age of the patient, especially in very young or elderly patients. Pathophysiology in elderly patients with hemodynamically significant ASD shows the nature of the problems to be very complex; such problems are frequently combined and usually interacted negatively. Nevertheless, transcatheter ASD closure may be potentially subtracted from the treatment option because the risk-benefit relationship of the procedure has been less-clearly defined in this age group. In this issue, the article by Woo et al. reported the safety and efficacy of the transcatheter closure of ASD in elderly patients. In spite of the small cohort of patients, this study also demonstrated the substantial risk and complications frequently encountered in elderly patients with ASD. All of their patients who successfully underEditorial

Transcatheter Closure of Secundum Atrial Septal Defect in Patients Over 60 Years Old

BACKGROUND AND OBJECTIVES Atrial septal defect (ASD) is the one of most common congenital heart diseases detected in adults. Along with remarkable development of device technology, the first treatment strategy of secundum ASD has been transcatheter closure in feasible cases. However, there are only a few publications regarding the results of transcatheter closure of ASD in elderly patients, esp...

[Transcatheter closure of atrial septal defect].

Isolated secundum atrial septal defect is one of the most common congenital heart defects. Surgical closure is the treatment of choice but is associated with a chest scar, some morbidity and a relatively long recovery and the use of cardiopulmonary bypass. Transcatheter closure of secundum atrial septal defect is therefore an attractive approach. 3 children, aged 5-10 years, underwent successfu...

Integrated Percutaneous Atrial Septal Defect Occlusion and Pulmonary Balloon Valvuloplasty

Introduction: Atrial Septal Defect (ASD) is one of the most common congenital heart diseases during childhood. Today, ASD closure is done by occlutech device via cardiac catheterization. ASD repair with transcatheter technique has shown high closure rate. However, coexistence of severe pulmonary stenosis and large defect in atrial septum is rare. When these two problems coexist, the result is i...

The benefit of atrial septal defect closure in elderly patients*

OBJECTIVE Closure of an atrial septal defect in elderly patients is controversial. The aim of the study was to evaluate the outcomes of transcatheter closure of secundum atrial septal defects (ASDs) in elderly patients. PATIENTS AND METHODS From a total of 488 patients with ASDs who underwent transcatheter closure, 75 patients aged over 60 years (45 female, 30 male) with a mean age of 65.3±15...
